腹腔镜一期手术治疗双侧肾上腺肿瘤的安全性和疗效分析

摘　　要：目的：探讨腹腔镜一期手术治疗双侧肾上腺肿瘤的安全性和疗效.方法：对17例34侧腹腔镜一期手术治疗双侧肾上腺肿瘤患者的一般情况、手术方法及效果进行回顾性分析.结果：行腹腔镜双侧肾上腺全切术3例，一侧肾上腺全切和另一侧肾上腺部分切除术7例，双侧肾上腺部分切除术7例.平均手术时间136(105~185)mi n ，平均术中出血125(88~260)ml ，平均术后住院7(5~8)d .术中无大血管或脏器损伤等严重并发症发生，术后切口感染1例.3例双侧肾上腺全切患者术后均行终身激素替代治疗.术后平均随访16(3~36)个月.高血压等症状消失或较术前好转，未见肿瘤复发.结论：腹腔镜一期手术治疗双侧肾上腺肿瘤安全、可行.To evaluate the safety and feasibility of laparoscopic bilateral simultaneous adrenalecto my for the treatment of the bilateral adrenal tumors .Methods :In this study ,17 cases of bilateral adrenal tumors under-went laparoscopic bilateral simultaneous adrenalecto my .The preoperative characteristics ,the outcomes and compli-cations were analyzed .Results :Three cases received bilateral total adrenalecto my ,7 cases received bilateral partial adrenalecto my ,and 7 cases received total adrenalecto my in one side and partial adrenalecto my in the opposite side . The average operating duration was 136 min (range :105 to 185 min ),average blood loss was 125 mL (88-260 mL) and average postoperative hospital length of stay was 7 days (range :5-8 days ).Wound infection occurred in one case .Three patients subject to bilateral total adrenalecto my were given postoperative life long steroid replacement . During an average follow- up period of 16 months (range :3-36 months ),all cases were cured .Conclusions :Laparo-scopic bilateral simultaneous adrenalecto my is safe and effective for the treatment of bilateral adrenal tumors .
