摘  要:目的探讨2型糖尿病(T2DM)患者血清胰高血糖素样肽-1(GLP-1)水平变化与胰岛细胞功能的关系。方法 20例初诊T2DM、22例长病程(≥5 a)T2DM和18例健康对照者,测定空腹及餐后30、120 min血清葡萄糖、胰岛素、C肽、GLP-1水平,以稳态模型胰岛素分泌指数(HOMA-)、早相和晚相胰岛素分泌指数评价细胞功能。结果 T2DM组餐后血清GLP-1、C肽均较对照组低(P<0.01),尤以长病程组明显(P<0.05)。T2DM组血清GLP-1与血糖呈负相关,但与C肽、HOMA-、早相和晚相胰岛素分泌指数呈正相关。结论 T2DM患者血清GLP-1水平下降可能与胰岛细胞分泌功能减退有关。Objective To investigate the relationship between glucagon-like peptide-1(GLP-1) and pancreatic a-cell function in patients with type 2 diabetes mellitus(T2DM). Methods Fasting and postprandial serum levels of glucose, insulin, C-peptide and GLP-1 at 30 min and 120 min were detected in 20 patients with newly diagnosed T2DM, 22 T2DM patients with long duration(≥5 years), and 18 healthy controls. a-cell function was evaluated using HOMA-a, early-phase and late-phase insulin secretion. Results Postprandial levels of GLP-1 and C-peptide were lower in T2DM group than those in control group(P<0.01), especially in long duration T2DM group(P<0.05). Serum GLP-1 content was negatively correlated with blood sugar, but positively with C-peptide, HOMA-a, early-phase and late-phase insulin secretion. Conclusion Downregulation of serum GLP-1 level may be related to the secretory dysfunction of β-cells in T2DM patients.
