改进牛舌模型在产科会阴损伤修复综合能力培训中的应用

摘　　要：目的:提高助产士及产科医生对各种会阴损伤识别及修复的能力。方法:对美国哈佛医学院MarkSauerwein博士等发明的牛舌模型进行制作改进,开发其模拟各种复杂会阴损伤及修复的功能。利用改进的牛舌模型对本院24名助产士及低年资产科医生进行会阴损伤修复综合能力培训,培训前后用自制会阴损伤修复综合能力考评表对受训者进行评估与考核,通过培训前后分值的比较,观察培训效果。结果:培训前后的自我测评及操作考核显示,培训对象对复杂会阴裂伤、阴道壁血肿、缝合线穿透直肠壁等异常情况的识别、处理能力均有显著提高(P<0.01),对常规的会阴切开缝合技能也有较大改进(P<0.05)。结论:改进牛舌模型并用于助产士及低年资产科医生在产科会阴损伤修复综合能力的培训,效果满意。[Objective]To improve identification and repair ability of all kinds of perineal damage of obstetricians and midwives. [Method]Improve the cow tongue model invented by Dr MarkSauerwein from Harvard medical school of American and develop the simulation capabilities of complex perineal damage and repair.24 junior midwives and obstetricians were trained using the improved cattle tongue model,following by assessment and evaluation of their comprehensive capacity with assessment list.[Result]The capacity of identification and treatment of abnormalities such as complex perineal laceration,vaginal wall hematoma,suture through the rectal wall,was significantly improved(P < 0.005).And the routine episiotomy suturing skills also were improved significantly(P < 0.05).[Conclusion]The improved cattle tongue model can be used to improve the identification and repair ability of perineal damage of obstetricians and midwives.
