湖北省某三甲医院3种手术单病种住院费用构成及影响因素分析

摘　　要：目的分析某三甲医院3种手术单病种住院费用构成及影响因素,寻找控制单病种医疗费用不合理增长的措施。方法使用构成比分析湖北省某三甲医院2005～2009年3种手术单病种住院费用的构成,使用累积Lo-gistic回归模型寻找费用的影响因素。结果实际住院天数是这3种手术单病种住院费用共同有统计学意义的影响因素,且实际住院天数长的病人住院费用倾向于高费用段。结论应明确诊断和手术名称的界定与书写,合理编码;降低手术单病种的药费比例;缩短实际住院天数。Objectives To analyze the proportion and influencing factors of hospitalization costs of three single operative diseases in certain tertiary hospital in Hubei province and to find some expense control methods.Methods The proportion analysis was used to analyze hospitalization costs constitution of three single operative diseases from 2005 to 2009.The cumulative logistic regression was used to find influencing factors.Results The days of hospital stay was the conjunct influencing factor in statistical significance.The more days of hospital stay,the higher hospitalization costs.Conclusions The paper put forward some suggestions such as clear definition and recording the diagnosis and operation and code rationally,decreaseing the proportion of medicine in single operative diseases and shortening the days of hospitalization.
