北京市两家大型医院检验项目与LOINC术语的映射试验

摘　　要：目的旨在建立北京市两家大型医院本地检验项目与LOINC代码之间的映射关系,为区域医疗信息平台互操作技术验证与实现项目做前期准备。方法本地检验项目字典数据分别来自北京市参与该项目的两家大型医院;LOINC数据库来自LOINC委员会;映射工具为RELMA映射软件;对两部本地检验项目字典进行分析和预处理,并将其导入映射工具;将两部字典之中的检验项目与LOINC代码之间进行映射,并对映射结果进行总结分析。结果两部字典之中的大多数检验项目均可找到相应的LOINC代码;成功映射的比例类似于国外报道。结论研究表明,为两家医院本地字典的检验项目与LOINC代码之间建立映射关系是切实可行的;需要建立健全医学术语服务机制,培养和配备胜任映射工作的编码人员,不断改进本地检验项目字典、LOINC,以及它们之间的映射关系,从语义互操作的层面上,促进和成就检验结果在不同医疗机构或信息系统之间的交换与共享。Objective To tentatively map laboratory tests from two large hospitals in Beijing to LOINC(Logical Observation Identifiers Names and Codes) terms,in order to prepare for the Project of Technical Validation and Implementation of the Interoperability of the Regional Healthcare Information Sharing Platform.Methods Two local data dictionaries of laboratory tests were provided by two large hospitals in Beijing,respectively,for the Project.The LOINC database was provided by the LOINC Committee.The RELMA(Regenstrief LOINC Mapping Assistant) software was utilized to complete the mapping task.The local data dictionaries were analyzed,preprocessed and then imported into the mapping tool for further mapping to LOINC terms.Finally,the resulting mappings were reviewed and evaluated. Results The suitable LOINC terms can be found for the majority of these local laboratory tests.And the proportions of the successful mappings are similar to the results in the international reports.Conclusion Our study shows that it is feasible to mapping the laboratory tests in the local data dictionaries of the two hospitals to LOINC terms.Furthermore,in order to facilitate and enable the interchange and sharing of clinical laboratory results among disparate healthcare organizations and information systems at the semantic interoperability level,a medical terminology service mechanism needs to be developed and improved;skilled professional coders need to be trained and assigned for the mapping tasks;and it is also necessary to continually improve the quality of local data dictionaries of laboratory tests,the LOINC database and the mappings between them.
