老年人脊椎结核的MRI诊断

摘　　要：目的探讨老年人脊椎结核的MRI表现及其诊断价值.方法回顾性分析12例经手术病理或临床证实的老年脊柱结核患者的MRI资料,观察椎体、椎间盘、附件破坏、硬膜囊和脊髓受压情况.结果12例患者36个椎体受累,相邻2个椎体受累6例,相邻3个椎体受累1例,相邻4个椎体受累4例,相邻5个椎体受累1例.病变椎体在T1WI上呈低信号或混杂信号,T2WI上呈高信号或混杂高信号,增强扫描后呈不均匀强化.结论MRI能清楚显示老年人脊椎结核的椎体骨炎、椎间盘破坏程度、附件受累情况、椎旁脓肿及脊髓损害情况,对老年人脊椎结核的诊断特别是早期诊断、鉴别诊断有确切价值.Objective To investigate the elderly spinal tuberculosis and its diagnostic value of MRI manifestations.Methods The MRI features of 12 elderly patients with spinal tuberculosis proved by pathology or clinic were reviewed.The MRI changes of vertebrae,intervertebral disc,attachment damage,dural sac and spinal cord compression situation were observed.Results 36 vertebrae in 12 patients were involved,two vertebrae in 6 patients,three vertebrae in 1 patient,four vertebrae in 4 patients,5 vertebrae in 1 patient.Damaged vertebrae showed low signal or mixed signal on T1WI,high signal or mixed with high signal on T2WI,and enhanced scanning were heterogeneous enhancement.Conclusion The MRI can clearly demonstrate osteitis,the damage degree of intervertebral disc,destruction of attachment,abscess and spinal cord damage.It has the exact value in the diagnosis of spinal tuberculosis in the elderly,especially the early diagnosis and the differential diagnosis.
