摘　　要：目的分析河池市5～60岁居民死亡主要原因,分析口头尸检(VerbalAutopsy,VA)进行死因调查的效果。方法建立死亡监测系统,对所有报告的监测区内5～60岁死亡个案进行口头尸检调查。结果2002年7月—2003年6月金城江镇、东江镇共死亡624人,全人口总粗死亡率为4.67‰,标化死亡率为5.34‰,男女之比为1.71∶1;5～60岁死亡病例248例,粗死亡率为2.10‰;导致5～60岁居民主要死亡原因为意外死亡、恶性肿瘤、心脑血管疾病、消化系统疾病等,其中意外死亡占死亡构成比的1/3;通过VA进行死因调查,使3.00%的错误死因定义得到更正、5.00%的不明死因得到诊断;无因伤寒导致的死亡。结论交通事故成为当地5～60岁居民死亡的主要因素;随着社会经济发展,伤寒等肠道传染病已不是导致死亡的重要因素;使用VA对于死因的判定比一般的监测系统报告更为准确。OBJECTIVE To analyze the cause of death in 5～60 years population in Hechi city and the result of assessment of cause of death by using verbal autopsy.METHODS Establishment of surveillance system and verbal autopsy was applied to determine all death cases aged 5～60 years in study area.RESULTS From 2002 July to 2003 June,total 624 death cases in all ages were reported in Jin chengjiang and Dongjiang towns.The crude death rate and standard death rate of all age were 4.67‰and 5.34‰respectively and the ratio of male to famale were 1.71∶1;the death cases aged 5～60 years were 248 and the death rate was 2.10‰;the main death causes in the 5～60 years old population were accidents and their adverse effects,neoplasm,diseases of circulatory system,diseases of digestive system;the proportion of accidents and their adverse effects accounted for one third of total death cases aged 5～60 years;by applying verbal autopsy to determine death cause,3%of ill defined causes were corrected and 5%of unknown causes were determined;no death cause resulted in typhoid fever.CONCLUSIONS Traffic accidents are main death cause in 5～60 years population;along with ecnomic development,typhoid fever and intestines diseases are not the main causes leading to death;it is more correct by applying verbal autopsy to determine death cause than usual surveillance system.
