播散性组织胞浆菌病的临床及影像学表现

摘　　要：目的 :分析讨论播散性组织胞浆菌病的影像学表现及价值 .方法 :回顾收集 3 2例播散性组织胞浆菌病的临床及影像学资料 (包括胸部X线及CT、B超 ) .结果 :胸部影像异常表现 1 9例 ,表现为 :( 1 )肺间质炎症 ;( 2 )渗出性肺实变 ,可伴胸腔积液 ;( 3 )肺门淋巴结肿大 ;( 4 )结节增殖样病灶 .肝脾肿大 3 2例 .结论 :播散性组织胞浆菌病表现呈多样性 ,尽管缺乏特异性 ,但影像学在诊断、鉴别诊断、对病变分型及了解病变范围和程度上有重要意义 .Objective:To study the imaging manifestations and clinical values of progressive disseminated histoplasmosis(PDH).Methods:Analyzed were 32 cases of PDH confirmed by marrow biopsy collected from 1989 to 2003.Every case had image check including X-ray,CT and B-ultrasound imaging.Results:19 out of 32 chest image checkings revealed different abnormal symptons:acute interstitial pneumonia,acute small lobour pneumonia with possible hydrops in thoracales,lymph nodules’enlargement in hilar of lung or nodular hyperplasia foci in chest cavity.32 abdomen imagings had liver and spleen enlargement.Conclusions:Though histoplasmosis does not have specific signs,imaging findings of PDH are of significance for various histoplasmosis diagnosing,ensuring its types,its lesions extension and its degree.
