抗环瓜氨酸肽抗体及类风湿因子联合检测在类风湿关节炎诊断中的应用价值研究  被引量:2

摘  要:目的探讨抗环瓜氨酸肽(CCP)抗体和类风湿因子(RF-IgM)联合检测对诊断类风湿关节炎(RA)的价值,评价其在鉴别RA和其他结缔组织疾病(非RA组)中的意义。方法采用酶联免疫吸附试验(ELISA)和散射比浊法分别检测89例RA患者、102例其它结缔组织疾病患者(弥漫性结缔组织病52例、血清阴性脊柱关节病17例、痛风17例、骨性关节炎12例、干燥综合征4例)及80例健康人的抗CCP抗体及RF含量。结果抗CCP抗体诊断RA的特异性和敏感度分别为94.12%和75.28%。RF诊断RA的特异性和敏感度分别为51.96%和89.89%,抗CCP抗体诊断RA的特异性比RF高(p<0.01),RF诊断RA的敏感性比抗CCP抗体高(p<0.05)。结论抗CCP抗体和RF联合检测可以提高RA诊断的敏感性和特异性,有助于RA与其他结缔组织疾病的鉴别。Objective To examine the diagnostic significance by detection anti-cyclic citrullinated peptide(CCP) antibody and rheumatoid factor(RF-IgM),and to examine the value in differentiating patients with rheumatoid arthritis(RA) from other rheumatic diseases(non-RA).Methods we used enzyme-linked immunosorbent assay and rate nephelometry to detect CCP and RF in patients with 89 RA、102 non-RA and 80 normal control group.Results The specificity and sensitivity for CCP were 94.12% and 75.28%,and for RF were 51.6% and 89.89%.the specificity was higher for CCP than for RF(p<0.01),and The sensitivity was higher for RF than for CCP(p<0.05).Conclusion The combination of anti-CCP and RF can raise the diagnostic accuracy.They were satisfactory markers in discriminating between RA and other rheumatic diseases.
