会阴B超在压力性尿失禁手术前后应用的价值

摘　　要：目的:探讨超声在女性压力性尿失禁(SUI)手术前后的应用价值及临床意义。方法:对26例患有中重度压力性尿失禁的女性患者,分别在安静状态下和Valsalva动作时,采用经会阴超声检测患者膀胱颈的移动度(Mu)、膀胱颈至耻骨联合后缘中点的距离(Br、Bs),尿道膀胱后角(ar、as)和膀胱颈的旋转角度(θ)。应用尿道中段悬吊术,即经阴道无张力尿道吊带(TVT)手术15例,经闭孔尿道悬吊带(TOT)手术11例。术后再次进行相应的检查。方法:手术后尿失禁的症状均得到缓解,各项超声指标恢复或改善,3例出现残余尿阳性。结论:结论:1、超声检查在女性SUI的诊断、术后的随访等方面有重要的意义。2、两种手术对治疗SUI有相同的疗效。Objective:To assess the role of perineal sonography and its clinical significance in preoperative and postoperative application in women with stress urinary incontinence.Method:Twenty-six patients with moderate and severe SUI underwent two defferent operations,including-15 in TVT and 11 in TOT.During perineal ultrasonography,distance from the bladder neck to the posterior midpoint of symphysis pubis(B),distance of bladder neck mobility(Mu),posterior urethra-vesical angle (ar,as) and the vesical neck rotational angle(θ) were all measured at rest and during stress before and after surgery.Results: After operation,The symptoms of stress urinary incontinence were relieved in all cases.Each ultrasound parameter was also improved. Three cases had residual urine postoperatively.Conclusion:1.Perineal ultrasonography plays an important role in SUI diagnosis and follow-up.2.The efficacy of the two kinds of suspender surgery is obvious.
