无创正压通气治疗COPD合并Ⅱ型呼吸衰竭疗效观察

摘　　要：目的观察无创正压通气治疗慢性阻塞性肺病合并Ⅱ型呼吸衰竭的临床疗效。方法COPD急性发作期合并Ⅱ型呼吸衰竭患者28例,予以常规治疗和无创正压通气治疗,监测治疗前和治疗后4、24、72h血气参数和心率,呼吸频率变化。结果28例中26例进行无创正压通气治疗,16例神志转为清楚,26例治疗后4、72h分别与治疗前比较及治疗后4与72h比较,心率、呼吸频率明显减慢,血气参数pH、PaO2、PaCO2明显好转。结论无创正压通气治疗COPD合并Ⅱ型呼吸衰竭疗效确切。Obejective To investigate the clinical effect of non-invasive positive pressure ventilation(NIPPV)in patients with chronic obstructive pulmonary disease(COPD)with Type Ⅱ respiratory failure.Method 28cases of type Ⅱ respiratory failure from COPD received non-invasive ventilation in addition to connetional medications.Arterial blood gases were measured and the change of clinical symptoms were observed before and after ventilation at 4hours、24hours and 72hours respectively.Results After 72 hours of non-invasive ventilation,16cases turn into conscionsness,arterial blood gases of 26 cases improved significantly,heart rate and respiratory rate slow significantly.Conclusion non-invasive positive pressure ventilation is an effective method for the treatment of type Ⅱ respiratory failure from COPD.
