摘 要:目的 探讨纯音测听、声导抗测试与仔细的鼓膜检查、电耳镜检查、鼓膜穿刺等方法对分泌性中耳炎(SOM)的诊断价值。方法 50例(80耳),其中男41例,女9例,<12岁15例(30耳),>12岁35例(单耳20例,双耳15例,共50耳);采用电耳镜、鼓气耳镜检查、纯音听阈测试、声导抗测试、鼓膜穿刺或鼓膜切开等方法对SOM的患者进行检查。结果 鼓膜颜色改变占88.8%,鼓膜检查对SOM正确诊断率为90%,言语频率气导听阈平均值>25dB,为86.3%,鼓室导抗图B型为88.8%,声导抗测试对SOM正确诊断率为93.8%,鼓室抽出浆液性液体为12.5%,粘液性液体为76.3%,胶耳为11.3%。结论 仔细的鼓膜检查和声导抗测试对SOM的诊断都很有价值。Objective To probe the value of pure tone checking,accoustic impedence and checking up tympanic membrane, electro-otoscopy, tympanotomy in diagnosis of SOM. Methods There were 50 patients who underwent the checks,41 patients were male and 9 patients were female. 15 patients (30 ears) were less than twelve years old,35 patients (50 ears) were more than twelve years old (20 patients were single ear, 15 patients were both ears). Method We can diagnose SOM patients by electro-otoscopy, siegle otoscopy, pure tone checking, accoustic impedence, and tympanotomy or myringotomy with grommet insertion. Results The ratio of colour change of tympanic membrane is 88. &% ,and the ratio of corrective diagnosing SOM was 90. 0%. The average threshold of air conduction of speech frequency which was more than 25 dB was more than 86. 3%. The ratio of B tympanogram was 88. 8% ,the ratio of corrective diagnosing SOM was 93. 8%. The ratio was 12. 5% when got serious fluid by tympanotomy,76. 3% when mucoid fluid,11. 3% when glue ear.Conclusions Checking up tympanic membrane and accoustic impedence were both useful very much to diagnosing SOM.
