摘 要:目的:验证波形钢板固定股骨骨折的生物力学特性。方法:采用7具人尸股骨标本骨折后采用波形钢板和普通钢板加以固定,对照比较了两种不同固定方式下轴向压缩、三点弯曲、扭转试验下的生物力学性能。结果:波形钢板的强度比普通钢板的强度大32%,刚度高33%,二者呈显著性差异(P<0.01)。同时试验证明:波形钢板固定的应力遮挡率小于普通钢板32.5%,有利于骨重建应力环境的改善,能加速骨痂生长。两种钢板本身的强度和刚度也显示了前者优于后者。证明采用波形钢板是符合生物力学原则的,临床应用波形钢板治疗股骨干骨折147例,经5~24个月随访,优良率达95.8%。结论:波形钢板是治疗股骨干骨折较好的内固定方法。Objectives:To test the biomechanical features of immobilization of the femur with wave plates. Methods:Femurs from 7 cadavers were fractured and then immobilized with wave plates and common compressive plates respectively.Comparisons were made about the biomechanical properties of these two types of plates by means of axial compression,three point bending and twisting test. Results:The wave plates were significantly different from the common compressive ones( P <0.01) in that the strength and the rigidity of the wave plates were respectively 32% and 33% greater.Experiments proved that the stress resistance rate of the wave plates was 32.5 % less than that of the common compressive plates,which contributed to the improvement of the stress conditions for the bone remodelling and quickened the growth of the callus,the wave plates were preterable to the common compressive plates,and hence it was in accord with the principle of biomechanics to adopt the wave plates.Wave plates were used in treating 147 patients with the fracture of the femoral shaft.After 3 ̄24 months of follow up,the rate of excellent recovery turned out to be 95.8%. Conclusions:The application of wave plates was a desirable method of internal fixation to treat the fracture of the femoral shaft,and therefore deserved to be further popularized in clinical practice.
