氯氮平的药物动力学与临床疗效的关系

摘　　要：目的：为了探讨氯氮平的药物动力学与临床疗效的关系。方法：对６８名精神分裂症患者单次服用２５ｍｇ氯氮平后血清药物浓度变化进行了动态观察，并根据氯氮平治疗６周后的ＢＰＲＳ评分结果，将病人分成临床疗效好组及临床疗效差组，比较两组之间单次用药及治疗４周后血清药物浓度，数据用ｔ检验。结果：精神分裂症Ⅰ型，服药后第４、５时点临床疗效好组血药浓度明显高于临床疗效差组，ｔ值分别为３．１７９和２．６４２（Ｐ＜０．０１，Ｐ＜０．０５），其余各时点及精神分裂症Ⅱ型患者服药后各时点临床疗效好组与临床疗效差组血药浓度比较差异无显著性（Ｐ＞０．０５）。治疗４周后，精神分裂症Ⅰ型临床疗效好组单次用药量血药浓度明显高于临床疗效差组，精神分裂症Ⅱ型两组之间比较差异无显著性。结论：单次口服氯氮平后的血药浓度和治疗４周后单位用药量血药浓度均与近期临床疗效有关。Objective:The purpose of this paper was to study the relationship between the pharmacokinetics and clinical effect of clozapine.Methods:The dynamic concentration changes of serum clozapine were examined in 68 schizophrenia patients after taking one single dose(25mg clozapine),and they were divied into well-resporsive guoup and bad-responsive group with BPRS after 6-week treatment,and the concentration of serum clozapine in two groups were compared after one single does and 4-week treatment dividually.The datum analysed with t-test.Results:The blood drug concentration in type Ⅰ patients was significantly higher in those with good response than in those with less reponse at 4th and 5th hour, T value were 3 179 and 2.642( P <0.01, P <0.05).No other significant differene was found among the other hous between the good response group and the less response group both in type Ⅰ and type Ⅱ patients. After 4-week treatment,the blood drug concentration of unit dosage was significantly higher in type Ⅰ patients with good response than those with less response and no significant difference was found between the two groups of type Ⅱ patients. Conclusion: There were correlativity between the clinical curative effect and the blood drug concentration after taking one single does of clozapie, as well as the blood drug concentration of unit dosage after 4-week treatment.
