摘  要:目的 探讨不稳定型心绞痛(UA)病人血浆氧化低密度脂蛋白(OX-LDL)和白细胞CD_(18)表达及血清可溶性细胞间粘附分子-1(sICAM-1)浓度的变化。方法 采用酶联免疫吸附法(ELISA),分别检测了46例健康人和56例UA病人心绞痛发作期和缓解后24h的血浆 OX-LDL浓度、白细胞 CD_(18)表达和sICAM-1浓度。结果 UA病人心绞痛发作期和缓解后24h血浆0X-LDL浓度、白细胞CD_(18)表达及sICAM-1浓度明显增高,与对照组比较差异有显著性(P<0.001,P<0.01)。心绞痛发作时血浆OX-LDL、白细胞 CD_(18)表达和sICAM-1浓度均明显高于心绞痛缓解后24h(P<0.001),且发作时和缓解后24h血浆OX-LDL与白细胞CD_(18)表达、sICAM-1之间呈正相关(P<0.001)。自发型心绞痛病人各指标增高较心肌梗死后型和劳力型心绞痛更明显(P<0.01)。结论 血浆OX-LDL和白细胞CD_(18)表达及sICAM-1浓度增高可能与UA的发病和病情变化有密切关系。Objective To explore the changes of Oxidized Low Density Lipoprotein (OX-LDL) , leukocyte CD18 expression and soluble Intercellular Molecule-1(sICAM-1) in patients with unstable angina (UA). Methods The changes of plasma OX-LDL,Leukocyte CD18 expression and sICAM-1 levels were measured in 56 UA patients and 46 controls using ELISA technique. Results The plasma OX-LDL,leukocyte CD18 expression and sICAM-1 levels were higher in UA patients than those in controls(P<0. 001,0. 01),and the increases of plasma OX-LDL,leukocyte CDig expression and sICAM-1 levels in angina attack period were more obvious than those in the nonattack period(P<0. 001). The leukocyte CD18 expressipn and sICAM-1 level were positively correlated with OX-LDL in UA patients during angina attack and non-attack period (P< 0. 001). The increases of the indicators were more obvious in spontaneous angina patients than those in exertional and postin-farction angina patients(P<0. 001). Conclusion The increases of plasma OX-LDL,leukocyte CD18 expression and sICAM-1 levels were closely correlated with the occurence of UA and degree of pathologic changes.
