EBV体外转化丙型肝炎病人PBMC中HCV持续存在的研究

摘  要:丙型肝炎病毒(hepatitis c virus,HCV)感染的一个主要特征是病程慢性化并迁延发展。近年越来越多的研究显示,HCV感染单个核细胞在疾病发展过程中起着不容忽视的作用,与干扰素治疗后丙型肝炎的复发关系密切。HCV不仅具有亲肝细胞性,也有淋巴细胞嗜性。The issue of infection of peripheral blood mononuclear cells (PBMC) by hepatitis C virus (HCV) has potentially important imp- lications, but is still debated. We have used hepatitis C patients' PBMC transformed by Epstein--Barr virus (EBV) to subculture in vitro to test the persistence of HCV in PBMC. EBV was used for infecting PBMC from four subjects with HCV positive and transforming PBMC into lymphoblasts capable of being propagated indefinitely. Then, positive and negative HCV RNA strands of the cultured cells and supernatants was detected by reverse transcriptase polymerase chain reaction (RT--PCR) every other month. HCV RNA and NS3 and NS5 of HCV protein were further tested in the cells and supernatants by using in situ PCR, immunohistochemistry SP method and flow cytometer (FMC). Of 4 lines of the patients' PBMC transformed by EBV (EBVTB) that we gained, one line of EBVTB had been cultured for one year and the other 3 lines for 6 months. HCV RNA positive strands persisted in the cultured cells all the while. It is interesting that the negative--strand RNA in EBVTB and the positive--strand RNA in supernatants were observed intermittently. These findings were further sub0tantiated by the detection of HCV genomes in EBVTB using in situ PCR. These observations were further reinforced by the expression of NS3 and NS5 in the cytoplasms and cell membranes by using immunohistochemistry and FCM. Our results offer strong evidence for persistence of HCV RNA in mononuclear blood cells. The EBVTB might be useful in analyzing the mechanisms of HCV persistence in PBMC.
