摘　　要：目的 :研究慢性心力衰竭 (CHF)病人生活质量 (QOL)的影响因素 ,提出延长生存期、提高QOL的措施和建议。方法 :采用现况调查方法 ,应用心衰QOL、家庭支持和综合自护能力评估量表 ,对心内科住院治疗的 2 0 4例CHF者进行问卷调查 ,所得有效数据应用SPSS软件包进行统计描述和Logistic回归分析。结果 :80 .9%的CHF者QOL处于中低水平。差异有显著性意义 (P <0 .0 5 ) ,使QOL呈下降趋势的因素有 :年龄、体力劳动者 ,心功能分级增高和心衰年限长伴CHD者 ,住院前不合理用药未及时应用非药物治疗方法行血流重建者 ,不做运动训练者 ,由配偶以外的子女等提供帮助者 ,家庭成员的行为和态度处于中低水平者 ,家庭人均月收入 <6 0 0元和医疗费自费者 ,综合自护能力处于中低水平者。结论 :影响CHF病人QOL的因素很多 ,4项对策可提高其QOL。Objective:To study the affecting factors on quality of life (QOL) in chronic heart failure (CHF),and to put forward measures and suggestions of elongating life span and improve QOL. Methods: The research was a descriptive study.A survey was carried out among 204 CHF patients in cardiac wards,with the collection of demographic data, medical information, Minnesota Living with HF Questionnaire, Modified Procidano Family Support Scale, Modified HF Patients' Comprehensive Self-care Scale. Logistic regression analysis was run for all valid data on affecting factors of QOL using the SPSS package. Results: 80.9% of QOL of the surveyed CHF were in the low or middle range,with a notable difference ( P <0.05).Factors making QOL assume a declining trend were :age, manual labors, the higher cardiac function class and the longer time limit of CHF, especially with CHD, using drug unreasonably before being hospitalized, deficiency of usage rate for ACEI and β-blocker, no using non-drug therapy timely to open or reestablish infarct-related blood vessel, no engaging in sports, supported by children or others instead of spouse, support degree or attitude in the low or middle from family members, family income of less than$600 RMB per month, paying expense by oneself,and comprehensive self-care abilityin the low or middle. Conclusion: Many factors affect QOL of CHF. Four measures may improve QOL in CHF.
