RF、MRF间接复位椎管内骨块治疗胸腰椎爆裂型骨折  

Treatment of Thoracolumbar Vertebrae Burst Fracture by RF/MRF Instrument with Indirect Reduction Bone in Spinal Canal

摘　　要：目的　观察 RF、MRF钉内固定系统间接复位对胸腰椎爆裂骨折的疗效。方法　对 36例胸腰椎爆裂骨折病人 ,后侧入路 ,RF、MRF钉内固定撑开 ,间接复位减压 ,植骨。结果　术后 X片、CT复查 ,椎体高度恢复 ,椎管内面积基本正常 ,脊柱矢状面生理曲度恢复 ,按 Frankel分级术后神经功能均恢复至 E级。平均随访 2 5 .5个月 ,无断钉及骨不愈合 ,无神经及脊髓继发损伤。结论　 RF、MRF撑开装置间接复位治疗胸腰椎爆裂骨折具有手术操作简便 ,时间短 ,出血少 ,不打开椎管 ,椎管扩大间接复位减压效果好 ,神经功能恢复好等优点 ,且重建了脊柱的稳定性 。Objective To observe the effects of RF/MRF instrument internal fixation with indirect reduction on the treatment of thoracolumbar vertebrae burst fracture.Methods 36 cases with thoracolumbar vertebrae burst fracture were surgically treated, RF/MRF instruments were internally fixed and spread, and pieces of bone were grafted in patients.Results After operation, X-ray films and CT scan demonstrated that the height of corpus vertebrae, the area of spinal canal and sagittal plane were restored.According to Frankel′s grading, nerve functions totally improved to E grade.The average period of follow up were 25.5 months.There was no case of damaged RF/MRF instrument or bone nounion.Conclusion The operation of RF/MRF instrument internal fixation has the advantages of simplicity, speediness, less bleeding, slight disturbance to spinal canal, and effective indirect reduction.In addition, the spinal stability is rebuilt, and the improvement of sagittal plane and nerve function is well obtained.
