血液病患者血清可溶性白细胞介素2受体水平及其临床意义

摘　　要：应用ELISA方法检测253例血液病患者血清可溶性白细胞介素2受体(sIL-2R)。结果显示,除多发性骨髓瘤(MM)外,各种造血系统恶性肿瘤患者血清sIL-2R水平均显著高于正常对照。急性白血病患者血清sIL-2R水平和外周血白细胞数及幼稚细胞百分比呈明显正相关,并影响预后;缓解期sIL-2R含量降至正常,但复发时又恢复至初诊水平。慢性髓细胞白血病(CML)慢性期轻度高于正常,但加速期和急变期升高明显。再障患者血清sIL-2R正常,区别于骨髓增生异常综合征(MDS),而免疫性疾病特发性血小板减少性紫癜(ITP)患者sIL-2R水平升高。因此,血清sIL-2R水平可作为鉴别疾病类型、监测疾病过程、反映机体免疫功能状况和判断预后的一个有用指标。The Ievel of soluble interleukin-2 receptor (sIL-2R) was evaluated in the sera of 253 patients with hematologic diseases using enzvme-linked immunosorbent assay (ELISA). The result showed that the Ievel of slL-2R in these patients was significantlv elevated as compared with that of normai controls (419.1 ±243.7 U/ ml), excepting multiple mveloma (MM, 538.7 ± 474.7 U/ml) and aplastic anemia (AA, 491.48± 298.18 U/ml) patients. Differing from AA, myelodysplastic sydromes (MDS, l 273.3±686.3 U/ml) had high sIL-2R level but lower than that of malignant lymphomas (l 450.2± 627.1 U/ml) and acute leukemia (1582.9±589.2 U/ml). Idiopathic thrombocytopenic purpura (ITP) had similar sIL-2R level (1143.1 ± 347.8 U/ml) as to MDS. The sIL-2R level in acute leukemia patients was positively correlated with the number of WBC and leukemia blasts in peripheral blood and af-fected prognosis. It decreased to normai Ievel during complete remission of acute leukemia (523.3±282.7 U/ml), but increased again when relapse occurred (1660.6±351.9 U/ml). CML patients in chronic phase had slightly higher serum sIL-2R level (834.0 ± 540.0 U / ml) than controls, but patients in accelerated phase (1625.0± 606.3 U/ml) and with blastic crisis (1620.0±411.3 U/ml) showed much higher levels. These results suggest that the serum level of sIL-2R might be a useful clinical indica-tor in monitoring different phase of blood diseases and prognosing AL, and could also reflect the condi-tion of body's immune function.
