摘  要:观察临床心衰患者和动物心衰模型心脏局部肾素-血管紧张素系统(RAS)和心钠素(ANF)的变化。结果显示:19例风心病患者心肌组织血管紧张素Ⅱ(ATⅡ)和ANF含量高于正常对照组,心肌ATⅡ量的变化与左室心肌重量呈显著正相关(γ=0.5723,P<0.05);动物心衰模型中,梗塞Ena组左室舒张末压、心室肌ATⅡ和ANF含量明显低于梗塞对照组,但左室收缩压和左室压力上升最大速率两两组间无显著性差异。提示:心脏局部RAS活性增高和ANF的变化参与心肌细胞增殖及影响心室重塑的意义较为突出,长期应用血管紧张素转换酶抑制剂,具有降低局部RAS活性,影响心室重塑和改善心功能的作用。Abstract :The cardiac local renin-angiotensin system (RAS) and Atrial natriuretie factor (ANF) were studied in patients and model rats with cardiac insufficiency. The results showed that the contents of myocardium angiotensin Ⅱ (AT Ⅱ ) and ANF in patients with chronic rheumatic heart valve disease were higher than those in normal and the AT Ⅱ correlated positively with the mass of left ventricular (γ=0. 5723. P< 0.05), Left ventricle end—diastolic pressure, myocardium AT Ⅱ and NAF in model rats of infarction enalapril group were significahtly lower than those in infarction control group and there were no difference of left ventricle systolic pressure and left ventricle maximal dp/dt between the two groups. The results indicate that activated cardia local RAS and ANF play more role in myocardium cell proliferation and ventricle remoduling and that long—term administration of enalapril improve cardjac function.
