彩色多普勒超声观察骨折骨痂血运的临床价值研究

摘　　要：目的　探讨彩色多普勒超声观察骨折骨痂血运的临床价值。方法　用彩色多普勒超声对 15例骨折患者在内固定术后第 1周、第 2周、第 3周、第 4周、第 9周及第 15周内依次观察患者骨折骨痂形成的声像特征及其血运情况 ,并与X线摄片进行对比分析。结果　骨痂形成良好的患者 ,二维超声示患者术后一个月内骨折处骨痂形成饱满 ,彩色多普勒血流显像示骨痂内部及周围有丰富的血流信号 ,血流指数RI <0 .6。而骨折骨痂形成不良者 ,超声示骨折间骨痂细小 ,彩色多普勒超声示骨痂内部及周边缺乏血流信号 ,RI >0 .7。结论　彩色多普勒超声可以作为一种早期诊断骨折骨痂愈合情况的方法 。Objective To investigate the value of color Doppler ultrasonography on fracture callus monitoring. Methods Fifteen patients with femur and/or tibial fractures treated with internal fixation were examined sonographically with color Doppler flow imaging (CDFI) during the 1st week, 2nd week, 3rd week, 4th week, 9th week, 15th week and about 6th month after the surgery when radiographic demonstration may confirmed the callus' consolidation. Results Twelve of fifteen patients had a well-developed callus, three patients showed delayed fracture healing. In patients with normal callus development, color Doppler imaging demonstrated the progressive formation of new vessels during the 1st month, especially during the 2nd, the 3rd,and the 4th week after the surgery with the blood flow resistance indices (RI) lower than 0.6 (RI<0.6); in the subsequent examinations, flow signals decreased, bone remodeling was confirmed by conventional radiography and gray-scale sonography. In contrast, lack of flow signals and persistent high RI (>0.7) were observed in the 3 patients with delayed fracture healing. Conclusion Color Doppler ultrasonography have the capability to predict whether the development of fracture calluses will be normal or delayed during its early rehabilitation time.
