核素血流灌注显像和B超诊断腹股沟隐睾的对比

摘　　要：目的　探讨核素睾丸血流灌注显像诊断腹股沟隐睾的诊断价值 ,并与超声检查结果比较。方法　 45例未能触及型隐睾患者 ,术前分别行核素睾丸血流灌注显像和B超检查 ,术后与病理检查结果对照 ,比较两组结果。另 10例正常对照者行睾丸血流灌注显像。结果　核素显像及B超对腹股沟隐睾诊断的灵敏度分别为 91%和 5 3 % ,准确率分别为 93 %和62 % ,两者均有极显著性差异 (P <0 .0 1)。核素显像和B超检查诊断腹股沟隐睾与病理诊断的阴性符合率分别为 90 %和3 3 % ,两者有显著性差异 (P <0 .0 5 ) ,阳性符合率分别为 93 %和 10 0 % ,两者未见显著性差异 (P >0 .0 5 )。Objective To evaluate the diagnostic value of testicular blood perfusion imaging of impalpable undescended testis. Methods Before surgery, 45 patients with impalpable undescended testis were studied with B-type ultrasonography and radionuclide testicular blood perfusion image. All results were compared with pathological finding. Results The sensitivity were 53% for B-type ultrasonography and 91% for radionuclide image,respectively.The accuracy were 62% for B-type ultrasonography and 93% for radionuclide image, respectively, they were all significantly different ( P <0.01). The positive coincident rate with pathological finding were 100% for B-type ultrasonography and 93% for radionuclide image, respectively, they are not significantly different ( P >0.05).The negative coincident rate were 33% for B-type ultrasonography and 90% for radionuclide imaging,respectively, they were significantly different ( P <0.05). Conclusion Testicular blood perfusion imaging should be considered as the first choice for preoperative localization of groin undescended testis when there is an unclear result on the B-type ultrasonograhy for it.
