改良式的逆行气管插管术

摘　　要：本文采用经环甲膜穿刺置人一根硬膜外导管牵引气管导管,另外再经环甲膜置入一根软管到气管内作高频通气,在配合使用疏喷妥纳和司可林的情况下,先后为11例插管困难的病人进行气管插管。并对其中6例病人分别于麻醉前、插管时和插管后10min采取动脉血作血气分析。作者认为:经环甲膜穿刺高频通气下逆行牵引气管插管方法安全,成功率高,创伤小,对于气管插管困难的病人有较好的实用价值。We describe a modification of retrograde guided intubation. With the help of transcricothyroid high-frequence jet ventilation, we used the epidural catheter to pull and guide the tracheal tube down the larynx and trachea, The tests of blood gas of six patients were done at the time of preanaesthesia, intubation and ten minutes after intubation. PaO_2of preanaesthesia, intubation and ten minutes after intubation were 77.72±21.17mmHg; 207.4 ±96.24mmHg ( P< 0.05, compare with preanaesthesia) and 213.7±119.14mmHg respectively. The technigue is safe, surprisingly fast, relatively atraumatic and easy to perform. It eliminates most causes of failure.
