^(99m)Tc-MIBI Myocardial Tomography: Clinical Comparison with Planar and Gated Myocardial Imaging

摘  要:本文报道了应用心肌血流灌注显像剂^(99m)Tc-MIBI对22例正常人、71例冠心病(CHD)患者的心肌显像结果。断层、门控与平面图像分析诊断CHD总的灵敏度分别为:91.5%、85.3%和80.9%。其中断层与平面显像比较有显著差异(P<0.05),而门控心肌图像分析与乎面显像之间无明显差别(P>0.05);三者特异性相近(P>0.05),分别为90.9%、84.6%与90.5%。37例受检者门控心肌显像所观察到的左室室壁运动结果,与同时进行的门控心血池显像有良好的一致性,左室节段运动符合率达90.3%(167/185,P<0.001),且对左室收缩异常程度及的判断较血池显像更为清晰、准确、灵敏度高,有较高的临床实用价值。The purpose of this study was to evaluate the diagnosis efficancy of planar and tomographic myocardial imaging with ^(99m)Tc-MIBI (^(99m)Tc-2-methoxy-isobutyl-isonitril)in coronary heart disease(CHD),22 normal subjects and 71 patients with CHD undergone ^(99m)Tc-MIBI myocardial tomography were presented.Planar and/or gated myocardial imaging were performed in some of them.The overall sensitivities of tomographic, planar and gated imaging for diagnosing CHD were 91.5%,80.9% and 85.3%,respectively.There was remarkably difference only between tomography and planar imaging(P<0.05). Similar specificities were found on three methods(P>0.05). Segmental wall motion derived by ^(99m)Tc—MIBI gated myocardial imaging was compared with that by simultaneously performed gated blood—poll imaging.A high correlation was present (P<0.001).Analysis in 185 left ventricule segments revealed an overall agreement in 167/185(90.3%). In conclusion,these results showed: (1)^(99m)Tc-MIBI myocardial tomography had good sensitivity and specificity in the detection of CHD and (2) ^(99m)Tc-MIBI gated myocardial imaging not only could detect the extent and region of ischemia and infarction,but also observe the abnormal wall motion segments,which was useful in clinical diagnosis.
