摘 要:目的:探讨黄芪和川芎嗪注射液联用对慢性阻塞性肺疾病(chronicob-structivepulmonarydisease,COPD)患者血液流变学、血气指标的变化及肺功能的影响。方法:将60例COPD患者分为治疗组和对照组,每组30例。治疗组常规西药治疗基础上加用中药黄芪和川芎嗪注射液,对照组单纯常规西药治疗。两组用药前后分别测定血液流变学、动脉血气指标及肺功能值。结果:治疗组治疗后血液流变学指标比治疗前有明显改善,气血黏度和血浆黏度均显著降低(P<0.01),血细胞比容指数也有明显下降(P<0.05);治疗组血气分析氧分压值升高、二氧化碳分压(PaCO2)下降(P<0.01),显著优于对照组治疗后指标(P<0.05);肺功能测定,治疗组治疗后肺活量、最大通气量、第1秒用力呼气量、最大呼气量、平均最大呼气流量指标分别为:(1.96±0.61)L,(43.46±16.88)L/min,(1.36±0.46)L,(3.69±1.47),(0.95±0.41)L/s,明显高于治疗前犤(1.76±0.61)L,(37.46±18.45)L/min,(1.06±0.36)L,(2.54±1.42),(0.68±0.52)L/s犦(P<0.01),组间治疗后比较治疗组明显优于对照组(P<0.05)。结论:黄芪和川芎嗪注射液联用具有协同作用,能有效改善COPD患者血液流变学指标及肺功能。AIM:To explore the effect of the combination of membranous milkvetch root(MMR) and ligustrazine on the hemorheological,vigor indices and lung function in patients with chronic obstructive pulmonary disease(COPD). METHODS:Sixty COPD patients were divided into treatment group(n=30) and control group(n=30).Patients in the control group received routine western medicine treatment,and besides,patients in the treatment group received the injection of MMR and ligustrazine. Hemorheological,arterial vigor indices and lung function value were measured before and after treatment in both groups. RESULTS:After treatment,hemorheological indices in the treatment group were improved obviously as compared with those before treatment,serum and plasma viscosities were decreased significantly(P< 0.01),and erythrocyte sedimentation index was also decreased obviously (P< 0.05);The blood gas analysis showed that value of partial pressure of oxygen was increased,while that of carbon dioxide decreased(P< 0.01) in the treatment group,significantly better than those in the control group(P< 0.05).The lung function determination showed that vital capacity, maximal voluntary(MVV),forced expiratory volume in 1 s(FEV1),peak expiratory flow(PEF),mean maximum expiratory flow(MMEF) in the treatment group were(1.96±0.61)L, (43.46±16.88)L per minute,(1.36±0.46)L,(3.69±1.47) and(0.95±0.41) L/s respectively,obviously higher than those before treatment[(1.76±0.61)L, (37.46±18.45)L per minute,(1.06±0.36)L,(2.54±1.42) and (0.68±0.52) L/s](P< 0.01),and the indices after treatment in the treatment were obviously better than those in the control group(P< 0.05). CONCLUSION:The combination of MMR and ligustrazine injection has a synergistic action,and can effectively improve the hemorheoloigcal indices and lung function.
