体外震波碎石诱发心律失常的临床分析

摘　　要：分析102例泌尿系结石行体外震波碎石术后患者的心电图变化,其中,术前心电图正常者99例中,有29例(29.29%)出现室性期前收缩,且多为频发性;术前心电图异常者3例中,有偶发房性期前收缩2例,术后转变为频发性,且部分形成二联律,另一例 ST—T 等明显加重。所有术后心电图有异常改变或加重者均未经特殊处理而自行缓解。结果提示,体外震波碎石术可诱发和加重心律失常,因此,在对心电图异常和心脏病患者行体外震波碎石术治疗时应观察心电图变化和对心脏功能的影响。The electrocardiographic(ECG)features durig extracorporeal shock wave lithotripsy(ESWL)were ana- lyzed in 102 patients with urinary lithiasis.There were 29 cases(about 29.29%)associated with ventricular premature beat and recurrent ventricular premature beat in most of them in 99 patients with EG normality before the lithotripsy.In 3 patients with ECG abnormality before the lithotripsy,atrial premature beats occasionally were turned into ones recurrently, couping heat—beats on partial ECG in 2 cases and more serous at ST—T segments,etc.than before the lithotripsy in an- other.All patients'ECG changes were sponteneous remission or disappearance without particular treatment.The results showed that ESWL can inducce arrhymias or make it heavier,thus,the changes on ECG and the deffects on cardiac func- tion should be observed during ESWL for the patient with ECG abnormality or cardiac disease.
