胃癌放疗前后影像表现与疗效观察(57例分析)

摘　　要：本文对57例胃癌手术前经放射治疗及组织学分析结果,结合放疗前后X线表现进行综合分析认为:气钡双对比造影是首选检查方法;提出了胃癌放疗前病影X线分类及放疗后病影X线表现。并以此为据进行了疗效和预后估计的偿试;还提出了胃癌的溃疡型、肿块型和浸润型三种大体X线类型,其组织学以管状腺癌、低分化腺癌居多数,适合手术前放射治疗。混合型组织学类型以粘液腺癌居多数,不适合手术前放射治疗。This study analysed 57 patients with gastric carcinoma who underwent preoperative radiotherapy and histologic evaluation combined with X—ray findings before and after radiotherapy. We consider that the air—barium double contrast examination should be first selected. We proposed X—ray classification of gastric carcinoma before radiotherapy, that is: A type(mass type), B type(infiltration type). C type(ulcer type) and D type(admixture type). X—ray findings of lesion after radiotherapy, that is: 'a' expressions lesion shadow was approximte to normal; 'b' expressions lesion shadow became small obviously, ulcer became shallow distinctly. Mucosas around lesion became more and coarse with disordered structures, local gastric wall was stiff slightly; 'c' expressions lesion shadow became small a little, mucosas around lesion became thick partly, but most of them were broken. local gastric wall was rigid. 'd' expression lesion shadow had no change or enlarged. on the basis of this. We attempt to ovaluate the curative effect and prognosis. We also proposed that most of these three kind of X—ray classified gastric carcinomas ulcer type, mass type and infiltration type are tubular adenocarcinoma and low differentiation adenocarcinoma in histology, and they adept to preoperative radiotherapy. Most of admixture type stomach careinomas are mucoadenocarcinoma in histology and are not suitable for preoperative radiotherapy.
