胰管栓塞后胰岛B-细胞内分泌功能变化机理探讨

摘  要:对8只正常犬行α-氰基丙烯酸脂胰管栓塞术,术后3月对其中5只行胰腺组织学及超微结构观察,旨在探讨胰管栓塞后胰岛B-细胞内分泌功能减退之机理。光镜观察发现胰外分泌腺基本消失,纤维组织形成,间有炎细胞浸润,胰岛内纤维组织增生、水肿。电镜观察见胰岛内水肿,胶原纤维增生,B-细胞呈不同程度损害。结果表明,B-细胞绝对数量减少、残存B-细胞结构损伤和代谢障碍以及局部环境的改变系B-细胞内分泌功能减退的主要原因。Pancre atic duct obstruction (PDO) was performed with an a-cyanoacrylate glue in 8 healthy beagles. Pancreatic biopsies were taken in 5 of the animals. Biop sies were studied on histology and ultrastructure in order to reveal the mechanisms in the decrement of the endocrine function of pancreatic islet B-cells. The exocriae tissue was almost replaced by fibrodid with scattered inflammatory cells. Pancreatic islets were edematous and fibrosis. B-cells were damaged in different degrees. Our results show that the hypoinsulinisms are mainly due to the decrease of the number of the B-cells, the structural damages and metabolic disturbance of the residual B-cells, as well as the microenvironmen tdisorder in the pacreas islets.
