门诊开展甲状腺手术的可行性探讨

摘　　要：目的:探讨开展门诊甲状腺手术的可行性。方法:回顾性分析2011-03-2011-07在我院施行门诊甲状腺手术、术后短期在院观察69例患者的临床资料,分析患者的手术情况,留院观察时间及并发症发生情况。结果:69例患者均手术顺利,平均手术时间为(42.86±15.11)min,术中平均出血量为(11.58±5.40)mL,平均手术切口长度为(4.37±0.93)cm,术后平均留院观察时间为(47.04±11.04)h。术中仅2例放置引流管,1例为毒性甲状腺肿,另1例为慢性淋巴细胞性甲状腺炎,引流量总量为(45±5)mL,引流管均于术后2d内拔除。术后出现短暂喉返神经损伤者1例,术后1个月恢复正常,所有患者无术后出血、低钙性抽搐和切口感染等并发症。结论:合理掌握手术适应证及禁忌证,配合专业的门诊手术团队,开展门诊甲状腺手术高效可行。OBJECTIVE: To explore the feasibility,safety and ascendance of day-operation of thyroid in China and evaluate the criteria for patients' selection.METHODS: Retrospectively analyzed clinical data of 69 patients treated with thyroid surgery in the day operation room from 2011-03 to 2011-07.RESULTS: All the operations were successful.The mean operation time was(42.86±15.11) min,the estimated blood loss was(11.58±5.40) mL,the mean length of incision was(4.37±0.93) cm,and the mean postoperative hospital observation time was(47.04±11.04) h.No patient complained about hematoma,hypocalcemia and wound infection.Drainage was inserted in 2 patients,including 1 toxic goiter and 1 chronic lymphocyic thyroiditis.The drainage volume was(45±5) mL,and the drainage was removed 2 days later.CONCLUSION: Day-operation of thyroid is feasible and safe with fittest operative team and well definited criteria for patients' selection.
