基于标准化护理语言的社区老年高血压患者家庭访视效果分析

摘　　要：目的探索基于标准化护理语言的社区老年高血压患者的家庭访视效果。方法采用典型抽样的方法在石河子市的3个社区选取已确诊的老年高血压患者120例,采用随机数字表法分为两组。试验组60例,在社区卫生服务站进行定期健康教育的基础上,由研究者应用标准化护理语言(北美护理诊断协会护理诊断、护理结局分类与护理措施分类)对患者进行1次/2周,连续12次的家庭访视,30～50 min/次;对照组60例,由社区卫生服务站进行定期健康教育。通过建立电子病历,应用护理结局分类指标比较两组效果。结果家庭访视后两组健康知识和行为、功能健康、心理社会健康、感知的健康、家庭健康得分及血压比较差异有统计学意义(P<0.05);而生理健康得分及体质指数(BMI)值比较差异无统计学意义(P>0.05)。结论基于标准化护理语言的社区家庭访视能促进健康管理,提高患者健康水平,有效降低老年高血压患者血压,提高患者家属照护能力和促进高级实践护士的成长。Objective To analyze the effect of home visit applying standardized nursing language to the elderly with hypertension in community.Methods A total of 120 elderly people with hypertension were selected from three communities in Shihezi City through typical sampling,and randomly divided into test group and control group.A health education program was provided to the control group through community health center.Besides the regular health education,each patient in the test group received 12 home visits applying standardized nursing language.Each visit lasted 30 ～ 50 min,which happened every two weeks.The electronic health record was built for each participant,and nursing outcomes were compared between the two groups.Results The health knowledge and behavior,function health,psychosocial health,perceived health,family health,and blood pressure in test group were significantly better than those in control group(P < 0.05).However,there was no significant difference in physical health and BMI between the two groups(P > 0.05).Conclusion Home visit applying standardized nursing language can improve health management including control of blood pressure,the care ability of the caregivers and so,which can eventually improve the patient's health status.And the application of standardized nursing language can also improve the capability of the nurses.
