摘 要:目的探讨超声斑点追踪显像技术在轻度肺动脉高压患者左心室收缩功能评价中的应用价值。方法选取2010年12月—2013年1月在辽宁医学院附属第一医院住院的50例轻度肺动脉高压患者(病例组)和同期体检健康者30例(对照组),先进行常规二维及M型超声心动图,测量左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)及左心室射血分数(LVEF),再应用超声斑点追踪技术测量两组受检者左心室壁各节段各水平收缩期的最大纵向应变率、径向应变率和圆周应变率。计量资料比较采用t检验,相关性分析采用Pearson相关分析。结果两组受检者二维超声指标LVEDD、LVESD、LVEDV、LVESV、LVEF间差异无统计学意义(P>0.05);但病例组患者左心室壁各节段各水平收缩期最大纵向应变率、径向应变率和圆周应变率均较对照组降低,差异有统计学意义(P<0.05);相关分析显示,LVEF与左心室各节段最大纵向应变率及最大径向应变率呈正相关(r=0.60、0.65;均P<0.05)。结论即使轻度肺动脉高压患者常规二维超声检测结果正常,超声斑点追踪显像却发现患者已存在左心室收缩功能受损,故后者评价左心室收缩功能更敏感。Objective To assess the value of speckle tracking imaging in evaluating the left ventricular systolic function in patients with slight pulmonary hypertension(PH).Methods 50 patients with mild PH(case group) and 30 healthy people(control group) admitted to the First Affiliated Hospital of Liaoning Medical College from December 2010 to January 2013 were selected.Left ventricular end diastolic dimension(LVEDD),left ventricular end systolic dimension(LVESD),left ventricular volume during end diastole(LVEDV),left ventricular volume during end systole(LVESV) and left ventricular ejection fraction(LVEF) were measured by two-dimensional echocardiography.Peak systolic longitudinal strain,peak systolic radial strain and peak systolic circumferential strain of every segment and level of left ventricular wall were measured by speckle tracking imaging.t test was used to compare measurement data and Pearson correlation analysis was used to analyze correlation.Results The two-dimensional indexes(LVEDD,LVESD,LVEDV,LVESV,LVEF) between the two groups showed no statistically significant difference(P > 0.05).The peak systolic longitudinal strain,radial strain and circumferential strain of every segment and level of left ventricular wall were significantly lower in case group than in control group(P < 0.05).Correlation analysis showed that LVEF was positively correlated with the peak systolic longitudinal strain or radial strain of left ventricular wall(r = 0.60,0.65;P < 0.05).Conclusion The left ventricular systolic function was damaged in mild PH,and the speckle tracking imaging is more sensitive than two-dimensional echocardiography in evaluating patients with mild PH.
