血浆前清蛋白和红细胞锌水平与手足口病患儿病情及预后的相关性

摘　　要：目的探讨血浆前清蛋白(PA)和红细胞锌水平与手足口病(HFMD)患儿病情及预后的相关性。方法选取孝感市中心医院2010年3月—2011年8月门诊及住院的HFMD患儿817例,根据病情程度分为普通组684例,重症组133例,给予相应治疗,另选取同期体检健康者30例为正常组,检测治疗前后血浆PA和红细胞锌水平。结果治疗前重症组患儿血浆PA和红细胞锌水平较正常组和普通组均降低(P<0.05);正常组与普通组比较,差异均无统计学意义(P>0.05)。治疗后普通组病情加重者血浆PA水平较正常组、普通组痊愈者和重症组痊愈者降低;重症组痊愈者红细胞锌水平较正常组和普通治愈者降低(P<0.05)。结论手足口病患儿血浆PA和红细胞锌水平对患儿病情的变化和预后有很好的临床预警意义,血浆PA恢复较快,而红细胞锌水平恢复较慢。Objective To investigate the serum prealbumin(PA) and erythrocyte zinc levels in children with hand,foot and mouth disease(HFMD) and their correlations with the disease condition and the prognosis.Methods A total of 817 patients with HFMD treated in Central Hospital of Xiaogan from March 2010 to August 2011 were included in this study.According to the severity of the disease,the patients were divided into two groups,which were mild group of 684 cases and severe group of 133 cases.Another 30 healthy children,who came to our hospital for regular physical examination during the same period,were recruited as the control group.The plasma PA and erythrocyte zinc levels were detected before and after the treatment.Results Before the treatment,the plasma PA and erythrocyte zinc levels in the severe group were significantly lower than those in control group and the mild group(P < 0.05),and no significant difference on the two variables was noted between the control group and the mild group(P > 0.05).After the treatment,the PA level among those uncured patients in the mild group was significantly lower than those of the control group and the cured patients in the mild / severe groups(P < 0.05).And the erythrocyte zinc level of the cured patients in the severe group was significantly lower than those of the control group and the cured patients in the mild group(P < 0.05).Conclusion The concentration of serum PA and erythrocyte zinc can predict the prognosis of HFMD well.The serum PA could return to the normal level fast,while it takes longer time for the recovery of erythrocyte zinc.
