斧形皮瓣在修复眉区软组织缺损中的应用

摘　　要：目的 通过斧形皮瓣的探索和应用,探寻修复眉区皮肤缺损的最佳方法.方法 选择在沈阳军区总医院烧伤科治疗的不同原因眉区皮肤缺损病例23例,根据病变的性质彻底切除病损组织及一定范围的周围正常组织.沿缺损区的长轴方向在眉毛的上方设计斧刃在上方蒂在下方的斧头状皮瓣,使弧线尽量与眉毛自然弧线一致.蒂部的宽度与皮瓣最长弧线(斧刃)的比例以＜1:3为宜.切开游离皮瓣并将皮瓣向软组织缺损部位旋转推进,尖端和旋转点处根据需要去除多余的皮肤,保证无张力下分层缝合.结果 本组病例所有皮瓣成活良好,无1例出现皮瓣坏死和血运障碍,眉毛无移位变形.术后5 ～7d拆线.经6个月至3年随访,术区原发病变无复发,未见瘢痕组织增生,功能和外形均满意.结论 斧头状皮瓣设计灵活,弧线与眉线一致,张力分布均匀,遗留瘢痕不明显,适合用于修复眉区皮肤缺损.Objective To found out a suitable method for the repairing of the eyebrow area soft tissue defect based on research and the clinical application of the ‘Hatchet Skin Flap’.Methods All 23 patients with eyebrow area soft tissue defect was included.The skin flap was designed along the longitudinal axle of the defect and with the curve of the ‘Hatchet Skin Flap’ parallel to the curve of the eyebrow.The scale of the width to the length of the flap should not less than 1:3.The flap was dissected and transferred to the defected area.The unnecessary skin in the tip area of the flap was dissected.The skin flap was sutured in different layer and without any tension in the sutured area.Results All 23 skin flaps was survived well and without any necrosis and blood supply disorder.The eyebrow was tot be influnced.After 6 months to 3 years follow up all of the skin flap with a satisfactory appearance.Conclusion Having the advantages as easy to design,no influence to the eyebrow,well-distributed tension and without obvious scar,the ‘Hatched Skin Flap’is suitable for the repairing of the soft tissue defect in the eyebrow area.
