摘 要:目的探讨铜绿假单胞菌(PA)感染对烧伤患者外周血γδT细胞合成胰岛素样生长因子Ⅰ(IGF-Ⅰ)的影响。方法以2012年3月至2013年7月在九七医院南京军区烧伤治疗中心治疗的87例烧伤患者及20名健康者为观察对象,统计患者的烧伤面积,抽取其在伤后2周及创面痊愈后的静脉血,伴有PA感染的患者还抽取感染期静脉血;健康者抽取清晨静脉血,用QuantibriteTM PE bead试剂盒检测标本γδT细胞IGF-Ⅰ的表达。根据是否感染PA,将烧伤患者分为感染组(n=8)及未感染组(n=79),比较烧伤面积与PA感染之间的关系;将健康者设为对照组,比较γδT细胞IGF-Ⅰ的表达在3组间的差异,并建立多元回归方程。结果感染组的平均烧伤面积明显大于非感染组,即PA感染与烧伤面积关系密切,PA可减少烧伤患者外周血γδT细胞合成IGF-Ⅰ,两者之间的相关性显著(r=-0.906,P=0.017)。以烧伤面积(X1)、PA感染(X2)作为自变量,以感染期γδT细胞IGF-Ⅰ合成水平为因变量作回归分析,得到多元线性回归方程:y=5.553+0.219X1-0.165X2。结论 PA抑制γδT细胞合成IGF-Ⅰ,这是烧伤创面愈合的不利因素。由于γδT细胞合成的IGF-Ⅰ在全身起作用,说明局部创面的PA感染能成为全身创面愈合的不利因素。Objective To explore the change of insulin-like growth factorsⅠ production in burns patients peripheral blood γδT cells effected by pseudomonas aeruginosa infection. Methods Eighty-seven burns patients treated in Nanjing military region burns treatment centre of the 97th hospital during March 2012 to July 2013 and 20 nomal persons were observed. The burn areas on patients were recorded,and the blood of patients were drawn at different time such as 2 weeks after injury、wound healing period and infectious period. The nomal persons were drawn blood in morning. The insulin-like growth factors Ⅰ productions in peripheral blood γδT cells were checked by Phycoerythrin Fluorescence Quantitation Kit. According to whether the infection of pseudomonas aeruginosa,the patients were divided into infection group( 8 cases) and non-infection group( 79 cases),the relationship between burn areas and pseudomonas aeruginosa infection was studied. The nomal persons were taken as control group and the differences of insulin-like growth factorsⅠ's production among 3 groups were compared and a multiple regression equation was established. Results Pseudomonas aeruginosa infection had a close relationship with burn areas,the mean burn areas in infection group were more than that in non-infection group. Pseudomonas aeruginosa could reduce insulin-like growth factors Ⅰ' s production in burns patients peripheral blood γδT cells,and there were significant correlations between them( r =- 0. 906,P = 0. 017). By taking Pseudomonas aeruginosa( X1) and burn area( X2) as independent variables,insulin-like growth factorsⅠ's production in γδT cells as dependent variable,a multiple linear regression equation: y = 5. 553 + 0. 219X1-0. 165X2 was established. Conclusions It was a disadvantage of wound healing that pseudomonas aeruginosa can reduce insulin-like growth factors Ⅰ's production in γδT cells.
