摘　　要：目的研究分析组织瓣覆盖技术在尿道下裂手术及尿瘘修复手术中的临床效果。方法选取本院2011年2月～2013年1月收治的58例尿道下裂手术及尿瘘患者,采用多种组织瓣覆盖技术对尿道和尿瘘内口实施覆盖技术,将其分为两组,其中,甲组尿道下裂27例,8例患者采用局部阴囊推进肉膜瓣法接受治疗,9例患者采用带蒂鞘膜瓣转移法接受治疗,10例患者采用带蒂背侧皮下组织瓣转移法治疗;尿瘘组患者31例,采用局部U形组织瓣法6例,局部X-V皮瓣法18例,带蒂背侧皮下组织瓣转移法7例,研究分析其治疗效果。结果实施治疗后,对58例患者随访2个月～3年,58例患者治疗效果满意,其治疗成功率为96.6%。术后尿道狭窄3例,阴茎曲矫正不满意4例。结论对尿道下裂手术及尿瘘患者,采用尿道下裂手术及尿瘘修复手术,可有效提高手术成功实施率,降低术后并发症的发生率,提高患者的生命质量。Objective To study and analyze the clinical effects of subcutaneous flap and tunica vaginalis flap technique in hypospadias repair and urinary fistula repair.Methods 58 cases of hypospadias and urinary fistula which were selected in our hospital from February 2011 to January 2013 following surgery were included.The new urethras or closed fistulas were wrapped with various types of subcutaneous flaps or tunica vaginalis flaps.All these participants were divided into two groups,hypospadias group in 27 cases,adjoining pedicle advance dartos flap technique was used in 8 cases,tunica vaginalis flap technique in 9 cases,and dorsal dartos flap technique in 10 cases.Urinary fistula group in 31 cases,adjoining U-shaped subcutaneous flap technique was used in 6 cases,X-V skin flap technique in 18 cases,and dorsal dartos flap technique in 7 cases.The therapeutic effects in both groups were analyzed and studied.Results After treatment,all the cases were followed up for 2 to 36 months with satisfactory effects.The success rate of operation was 96.6%.There were 3 cases presented with postoperative urethrostenosis and 4 cases with dissatisfaction of correction of penis curve.Conclusion In the hypospadias and urinary fistula patients,use therapy of hypospodias repair and urinary fistula repair techniques can effectively improve the success rate of operation,reduce the occurrence of postoperative complications,and better patient′s quality of life.
