提肛肌训练治疗经尿道前列腺电切术术后患者尿失禁的效果

摘　　要：目的探讨在手术前后持续进行提肛肌训练对经尿道前列腺电切术(transurethrue resection of prostate,TURP)后患者尿失禁的治疗效果。方法方便性抽样选择2011年2月至2012年2月收治南方医科大学附属南海医院泌尿外科进行TURP的患者240例,按照入院先后顺序随机分为观察组和对照组,每组各120例。观察组在术前、术后以及拔除尿管后均进行提肛肌训练;对照组只在术后拔除尿管出现尿失禁时进行提肛肌训练。对比两组患者术后发生尿失禁的概率及尿失禁的持续时间。结果观察组术后尿失禁的发生率为40.8%,明显低于对照组的86.7%,P<0.01;观察组发生尿失禁的持续时间为(4±0.5)d,明显少于对照组的(12±2.0)d,P<0.05;且观察组进行提肛肌训练的时间越长,术后发生尿失禁的概率越低。结论 TURP患者术前、术后以及拔除尿管后进行持续有效的提肛肌训练能预防或降低术后尿失禁的发生率,缩短尿失禁的持续时间,减少患者痛苦,提高患者的生活质量。Objective To explore the effect of pre-and post-operative continued levator ani muscles training on urinary incontinence after transurethrue resection of prostate(TURP).Methods From February 2011 to February 2012,240 patients undergoing TURP were randomly divided into experimental group(n=120) and control group(n=120) based on the admission sequence.Patients in the experimental group underwent levator ani muscles training strictly before and after operation and during removing the catheter;while patientsin the control group adopted levator ani muscles training only on urinary incontinence after removing the catheter.The rate of urinary incontinence after TURP and the period of urinary incontinence were compared.Results The therapy effect ratio of the experimental group was 40.8%,which was significantly lower than that(86.7%) of the contrel group(P<0.01).The period of urinary incontinence in the experimental group [(4±0.5)d] was significantly shorter than that(12±2.0)d of the control group(P<0.05).The longer the period of levator ani muscles training lasted,the lower the rate of urinary incontinence was.Conclusion Levator ani muscles training before and after operation and after removing the catheter is a good method to decrease the rate and to shorten the period of urinary incontinence after TURP,reduce patients' suffering and improve the quality of life.
