摘  要:目的探索宝石CT能谱成像(GSI)低管电压、低对比剂用量对活体肾移植肾动脉术前评估的应用价值。方法对2011年8月到2012年3月30例亲属活体供肾术前行螺旋CT血管成像,将其随机分为两组:常规组15例,常规扫描模式,管电压120 kVp,对比剂剂量为1.5 ml/kg;GSI组15例,应用GSI扫描,对比剂剂量为1.0 ml/kg。重建GSI组数据,选出右肾动脉开口水平腹主动脉与竖脊肌之间最佳对比噪声比单能量图像;对两组图像进行多种后处理技术,分别在原始图像上测量最佳CNR单能量图像和常规组混合能量图像中双肾动脉CT值、噪声、CNR,并以5分法比较两组重建图像的图像质量。结果 GSI组内,(53±2)keV单能图像具有最佳CNR,GSI组中最佳CNR单能量图像的肾动脉的平均CT值高于常规组的CT值。结论能谱CTA可减少对比剂剂量,同时提高血管内CT值,拥有常规CTA的图像质量。Objective To explore the gemstone spectral imaging(GSI) low tube voltage,low dose contrast medium for the evaluation of living renal transplantation renal artery before operation. Methods From August 2011 to March 2011,30 cases of relatives living donor kidney preoperative spiral CT angiography were randomly divided into two groups: Regular group(15 cases),the conventional scanning pattern,tube voltage 120 KVP,contrast medium dose was 1. 5 ml / kg; GSI group(15 cases),application of GSI scan,contrast medium dose was 1. 0 ml / kg. GSI group data were reconstructed,the best CNR single energy images of the right renal artery opening level were chosen between abdominal aorta and erector spinal muscle; a variety of post- processing techniques were used respectively on two groups of images,the best CNR single energy images and conventional group on the original image mixing energy image renal artery CT values,noise,CNR were measured,and the image quality of the image was compared using the five- point method. Results In GSI group,53 ± 2KeV single image had the best average CNR; CT of the best CNR single energy image of renal artery value in group GSI was higher than conventional CT value. Conclusion Energy spectrum of CTA can reduce the dose of contrast agent,while improving the intravascular CT value,with conventional image quality of the CTA.
