摘　　要：目的探讨肩胛骨外侧缘入路结合辅助切口、锁定重建钢板治疗肩胛骨骨折的临床效果。方法对14例OTA14A3型肩胛骨骨折患者采用双切口结合锁定重建钢板治疗。结果 14例均获随访,时间6～18个月。其中12例无肩部疼痛;13例患者肩部外展和屈曲>120°,患者外展肌力达到4+～5级;11例恢复伤前工作,3例能够完成家务。按Constant-Murley评分评价患肩功能:优8例,良4例,可2例。结论双切口结合锁定钢板治疗OTA14A3型肩胛骨骨折,术后康复锻炼可避免关节僵硬、疼痛等并发症,有效恢复肢体功能,临床疗效满意。Objective To study clinical effect of scapula lateral edge incision combined with auxiliary incision to reveal the operation area and fix with lock reconstruction plate.Methods 14 cases with OTA14A3 complicated scapular fractures were treated with double-incision approach and locking reconstruction plate.Results 14 cases were followed up for 6-18 months.Patients of 12 cases did not complain of any pain,the range of shoulder movement was greater than 120° in both abduction and flexion in 13 cases,and the abduction muscle strength was 4+-5 grade in all cases.11 cases returned to preinjury jobs,3 patients were able to do their housework.Functional evaluation was made according to the scoring system put forward by Constant and Murley.The results were excellent in 8 cases,good in 4,and fair in 2.Conclusions The double-incision approach combined with locking reconstruction plate is an effective operative method to treat OTA14A3 type scapular fractures.Early rehabilitation can prevent stiffness,pain and disability complicated by scapular fractures,and facilitate function restoration.It has satisfactory clinical effect.
