摘  要:目的 研究湖北地区汉族人群中类泛素样蛋白4(SUMO4)M55V基因多态性与2型糖尿病及其相关因素的关系.方法 采用聚合酶链反应-限制性片段长度多态性分析(PCR-RFLP)法对湖北地区2006年2月至2009年11月在武汉大学中南医院门诊及病房新诊断的427例2型糖尿病患者(糖尿病组)及281名健康对照者(对照组)进行SUMO4 M55V基因多态性检测,空腹测量身高、体重、腰围、血压,测定血糖、胰岛素、糖化血红蛋白(HbA1c)及甘油三酯(TG)等各项指标,并进一步探讨2型糖尿病及其相关危险因素和SUMO4 M55V基因多态性的关系.统计学方法采用Logistic多元回归分析评估与2型糖尿病发生相关的危险因素.结果 糖尿病组的腰围(t=2.72,P<0.01)、体重指数(BMI,t=2.66,P<0.01)、空腹血糖(t=2.81,P<0.01)、餐后血糖(t=3.07,P<0.01)、HbA1c(t=2.79,P<0.01)和甘油三酯(t=2.63,P<0.01)水平明显高于对照组.糖尿病组和对照组相比SUMO4 M55V的GG基因型分布频率显著升高(3个基因型比较:x2=6.782,P<0.05),且在人群中,携带GG基因型的患病风险是AA基因型的2倍(OR=2.011).Logistic多元回归分析显示,HbA1c(P≤0.05,OR=2.57,95%CI:1.92~4.63)、体重(P<0.05,OR=2.61,95%CI:1.74~4.37)、SUMO4M55V基因多态性(P<0.05,OR=1.47,95%CI:1.09~3.42)和TG(P<0.05,OR=1.59,95%CI:1.24~4.63)与2型糖尿病的发生明显相关.进一步对2型糖尿病不同基因型的HbA1c、BMI、TG、空腹胰岛素和胰岛素抵抗水平进行分析,发现在3个基因型之间TG差异明显(F=3.382,P<0.05),与AA基因型相比AG和GG基因型患者的空腹胰岛素(F=4.075,P<0.05)和胰岛素抵抗指数(HOMA-IR)(F=14.389,P<0.001)的水平也更高.结论 在湖北地区汉族人群中,SUMO4 M55V基因多态性和2型糖尿病发生风险相关.Objective To investigate the relationship between small ubiquitin-like modifer 4 M55V polymorphism with type 2 diabetes and related factors. Methods We analyzed the SUMO4 M55V polymorphism of 427 newly diagnosed type 2 diabetic patients, who were diagnosed firstly at outpatient and inpatient in Zhongnan Hospital of Wuhan University from 2006 February to 2009 September, with 281 healthy controls, by PCR-RFLP assay, and gathered their body height, weight, waistline, blood pressure on an empty stomach. Blood glucose, insulin and HbA1c also measured. Then analyzed the relationship between SUMO4 M55V polymorphism with type 2 diabetes and its related factors, with using logistic analysis for evaluating the risk factors of type 2 diabetes. Results Compared with the control group, the levels of waist circumference(t =2.72, P<0.01), BMI(t=2.66, P<0.01), FBG(t =2.81, P<0. 01), PPBG (t =3.07, P<0. 01)HbA1c(t =2.79, P<0.01) and TG(t =2.63, P<0. 01) are significantly higher in T2DM group( P <0. 01 ). The frequency of GG genotype was significantly higher in T2DM group( compare with the three genotypes: x2 =6. 782,P <0. 05), and the risk of type 2 diabetes is two times higher in GG genotype compared with AA genotype ( OR =2. 011 ). Logistic multifactor analysis found that there is notable association between type 2 diabetes incidence with HbA1 c ( P < 0. 05, OR= 2. 57,95% CI: 1.92 - 4. 63),body weight(P <0.05,OR=2.61, 95%CI: 1.74-4.37), SUMO4 M55V polymorphism (P <0.05,OR = 1.47,95% CI: 1.09 - 3.42), TG( P < 0. 05, OR = 1.59,95% CI: 1.24 - 4. 63 ). Further analysis of HbA1c, BMI, TG, FINS and HOMA-IR showed that there was significant difference of TG among different genotypes( F = 3. 382, P <0. 05), and compared with AA genotype, levels of FINS( F =4. 075, P <0. 05 )and HOMA-IR(F= 14.389,P<0.001) in AG and GG were also higher. Conclusion SUMO4 M55V polymorphism is associated with the incidence of type 2 diabetes among the Han population in Hubei.
