穴位埋线与耳穴贴压治疗中风后心脾两虚型失眠临床对照研究

摘　　要：目的:比较穴位埋线与耳穴贴压治疗中风后心脾两虚型失眠的疗效差异。方法:将60例中风后心脾两虚型失眠的患者随机分成埋线组和耳穴组,每组30例。埋线组接受穴位埋线结合针刺治疗,穴位埋线选取心俞、脾俞、厥阴俞;耳穴组采用耳穴贴压结合针刺治疗,耳穴选取交感、神门、内分泌、皮质下、心、脾、肝。治疗4个疗程之后进行疗效比较。以匹兹堡睡眠质量指数(PSQI)进行疗效评价。结果:治疗后埋线组总有效率为86.7%(26/30),优于耳穴组的76.7%(23/30,P<O.05)。两组均可改善PSQI各项评分(均P<O.05),埋线组入睡时间、睡眠效率及日间功能的改善优于耳穴组(P<0.05,P<0.01)。结论:穴位埋线治疗中风后心脾两虚型失眠的疗效优于耳穴贴压治疗。Objective To compare the efficacy difference of acupoint catgut embedding therapy and auricular point sticking therapy in treating insomnia due to heart and spleen deficiency after stroke.Methods Sixty patients with insomnia due to heart and spleen deficiency after stroke were randomly divided into a treatment group to receive combined acupoint catgut embedding and acupuncture therapy,the acupoint catgut embedding were treated with Xinshu(BL 15),Pishu(BL 20) and Jueyinshu(BL 14) and a control group to receive combined auricular point sticking and acupuncture therapy(30 cases in each group).The auricular point sticking was treated with Jiaogan,Shenmen,Neifenmi,Pizhixia,Xin,Pi and Gan.After 4 courses of treatment,the curative effect was compared.The Pittsburgh sleep quality index(PSQI) scale score was evaluated.Results The total effective rate was 86.7%(26/30) in the treatment group versus 76.7%(23/30) in the control group after treatment with a significant difference(P<0.05).According to PSQI.sleep duration,sleep efficiency and day time function in the treatment group were better than those in the control group(all P<0.05).Conclusion The effect of acupoint catgut embedding therapy for heart and spleen deficiency type of insomnia after stroke is better than that of auricular point sticking therapy.
