摘 要:目的探讨18F-2-氟-2-脱氧-D-葡萄糖(18F-fluorodeokyglucose,18F-FDG)符合线路SPECT显像技术在肺部阴影良恶性诊断中的价值。方法对1 113例肺部肿块患者进行18F-FDG符合路线SPECT检查以达到诊断目的,并与病理结果比较。结果 1 113例患者中,恶性疾病501例、良性疾病612例。病灶检出的灵敏度87.0%,特异度84.6%,准确率85.2%。其中鳞癌、腺癌、腺鳞混合癌和小细胞肺癌的准确率分别为86.9%、81.3%、95.5%和91.8%。结论 18F-FDG SPECT能有效显示肺癌病灶,在肺癌的诊断、良恶性肿瘤的鉴别中作为一种补充检查有重要的临床意义。该检查价格便宜,成像清晰,有良好应用价值。Objective To investigate the value of18F-fluorodeokyglucose(18F-FDG) imaging by coincidence circuit SPECT in the diagnosis of benign and malignant pulmonary shadows. Methods18F-FDG imaging by coincidence circuit SPECT was performed on 1 113 patients with lung lesions. The imaging results were compared with histological examination.Results There were 501 patients with malignant lesions and 612 patients with benign lesions. The sensitivity and specificity of18F-FDG for differential diagnosis were 87.0% and 84.6%, respectively. The accuracy rate was 85.2% in all, 86.9% in squamous cell carcinoma, 81.3% in adenocarcinoma, 95.5% in adenosquamous carcinoma, and 91.8% in small cell lung cancer. Conclusion18F-FDG imaging by coincidence circuit SPECT was effective in imaging lung lesions. It is of great value to be a complementary examination in the diagnosis and differential diagnosis of lung cancer. In addition, it is cheap and of good quality.
