纤维支气管镜直视下治疗非癌性难治性咯血90例临床效果观察

摘　　要：目的探讨采用纤维支气管镜直视下治疗非癌性难治性咯血的临床价值。方法选取2009年3月至2012年7月来我院治疗的非癌性难治性咯血患者90例,将纤维支气管镜插入气管开始治疗的过程中先对患者气管中的血栓及分泌物进行清除,组合患者的胸部CT片或者胸片找到病灶以及活动性出血的支气管,清除病灶附近的分泌物与血栓,在直视下对出血部位通过纤维支气管镜注入10mL生理盐水稀释后的5U凝血酶。结果本组90例患者在治疗后未出现气胸、低氧血症、心律失常、心衰或者喘息等并发症,均在24h内停止咯血。结论纤维支气管镜直视下治疗非癌性难治性咯血临床疗效显著,值得临床推广应用。Objective Explore the fiber bronchoscope under the direct treatment of the clinical value of non cancer refractory hemopty-sis.Methods Selection in March 2009 to July 2012 in our hospital treatment of non carcinoma of 90 patients with intractable hemoptysis ,in-sert the fiberoptic bronchoscopy airway start treatment in the process of thrombosis in patients with tracheal secretion and to remove and combi-nation in patients with chest CT slice or chest X-ray finding of bronchial lesions and active bleeding ,clear lesions near the secretion and blood clots ,under the direct of bleeding by fiber bronchoscope 10 mL saline injection diluted 5u thrombin.Results Does not appear after 90 patients in the treatment of pneumothorax ,hypoxemia ,arrhythmia ,heart failure or complications such as breathing ,all stop emptysis within 24 h.Con-clusion Fiberoptic bronchoscopy under direct non cancer intractable hemoptysis treatment clinical curative effect is distinct ,worthy of clinical popularization and application.
