床边超声引导徒手经皮肝胆道及胰周穿刺置管引流术的应用

摘　　要：目的探讨床边超声引导徒手经皮肝脏、肝内胆管、胆囊、肝周及胰周积液穿刺置管引流术的应用。方法对182例危重病人床边实施该术式并观察疗效。结果所有病人手术成功并引出大量感染性积液,未出现严重并发症。164例取得满意疗效,余18例死于晚期肿瘤或多器官功能衰竭。结论床边超声引导徒手经皮肝脏、胆道、肝胰周穿刺置管引流术方便、安全、高效。手术关键在于精确"找针"、"调针"以及"控制导丝"。Objective To evaluate the clinical application of percutaneous catheter drainage introduced by bedside ultrasonic guidance with free-hand technique in severe hepatobiliary and peripancreatic infection cases.Methods A total of 182 cases were treated with the operation and the efficiency was observed.Results A lot of infectious effusion was drained in all cases successfully without serious complications.A total of 164 cases received satisfactory outcome. Eighteen cases died from advanced malignant digestive tumor or MODS.Conclusion With advantages of convenience, safety,high efficiency and minimal invasion,bedside ultrasound-guided free-hand percutaneous catheter drainage benefits those severe hepatobiliary infection cases a lot.The key of the operational technique lies in finding and adjusting the needle accurately,and controling the guide wire well.
