摘  要:目的探讨CACNA1C基因多态性和体质量指数对小剂量氨氯地平降压疗效的影响。方法选择2013年1月~2013年6月在辽宁医学院附属第一医院治疗的86例高血压患者为研究对象,所有患者均采用左旋氨氯地平2.5 mg和治疗替米沙坦40mg治疗,每早起1次,2周后根据情况增加替米沙坦剂量到80 mg,4周后评价血压下降情况。检测患者CACNA1C基因多态性,计算体质量指数,分析患者体质量指数与临床疗效的关系。结果单因素分析结果显示低效组BMI显著高于中效组和高效组(P<0.05),收缩压和舒张压基线水平越高的患者降压效果越明显(P<0.05),携带rs2238032 GG型者下降幅度显著小于GT型和TT型(P<0.05)。多因素分析结果显示,rs2238032(GG)、基线收缩压、BMI是影响氨氯地平的降压疗效的影响因素(P<0.05)。结论 BIM和CACNAIC基因多态性影响到小剂量氨氯地平对高血压的降压作用。Objective To discuss the effect of CACNA1 C gene single nucleotide polymorphism and BMI on low-dose amlodipine for hypertension.Methods Clinical data of 86 cases with hypertension from Jan 2013 to Jun 2013 admitted in First Affiliated Hospital of Liaoning Medical were analyzed. All patients were treated by low-dose amlodipine and followed-up tharapy. Corralation between CACNA1 C gene single nucleotide polymorphism,BMI and clinical efficacy of low-dose amlodipine for hypertension were analyzed. Results Single factor analysis results showed that BMI of efficient group was higher than that of moderate effect and efficient group( P < 0. 05),and patients with high SBP and DBP basic line showed better clinical efficacy( P< 0. 05); clinical efficacy of patients with rs2238032 GG gene type showed worse than patients with rs2238032 GT and TT gene type. Results of multiple factor analysis showed rs2238032 GG gene type,SBP basical line,and BIM were influencing factors of clinical efficacy of low-dose amlodipine for hypertension. Conclusion rs2238032 GG gene type,BIM are influencing factors of clinical efficacy of low-dose amlodipine for hypertension.
