来曲唑和克罗米酚在原因不明不孕症患者宫腔内人工受精治疗的临床效果比较

摘　　要：目的比较来曲唑和克罗米酚在原因不明不孕症患者的宫腔内人工受精(IUI)治疗过程中的临床效果。方法回顾性分析2011年1月至2012年12月于本教学医院生殖医学中心接受IUI治疗的原因不明不孕症患者共201例。其中使用来曲唑促排卵的IUI周期173例,使用克罗米酚促排卵的IUI周期28例,比较两组促排卵效果和IUI临床结局。结果克罗米酚组患者促排卵后获得直径>14mm卵泡数较多,雌激素峰值较高。两组其他促排卵参数差异无显著性。来曲唑组临床妊娠率、活产率高于克罗米酚组,但该差异无显著性。结论在原因不明不孕症患者的IUI治疗中,来曲唑和克罗米酚的临床效果相近。Objective To compare the clinical effect of Letrozole and Clomiphene in ovulation induction with IUI cycles of the patients with unexplained infertility.Method This was a retrospective cohort study of the patients who underwent ovulation induction with IUI treatment, between Jan. 2011 and Dec. 2012, in a university hospital. A total of 201 IUI cycles were included. The Letrozole group included 173 IUI cycles, while 28 cycles with Clomiphene constituted as the Clomiphene group. Ovulation induction performance and IUI outcome were compared between the two groups.Result The number of follicles with diameter>14mm and the peak E2 level were higher in Clomiphene group compared with the Letrozole Group. No differences were found in other ovulation induction parameters. Patients undergoing ovulation induction with Letrozole and those with Clomiphene obtained comparable IUI outcomes, in terms of pregnancy rate and live birth rate.Conclusion This study suggests similar clinical effects of Letrozole and Clomiphene in patients with unexplained infertility.
