静息心率与收缩压乘积对中老年女性肱踝脉搏波传导速度的影响

摘　　要：目的探讨不同的静息心率与收缩压乘积(率压乘积)水平对女性肱踝脉搏波传导速度(Ba PWV)的影响。方法采用横断面研究设计,在2006年至2007年参加健康体检的101510例开滦集团职工中随机分层抽取5852例作为研究对象,进行统一问卷调查、血液生化及Ba PWV检查,符合入选标准的(年龄≥40岁、既往无缺血性脑卒中及短暂性脑缺血发作、心肌梗死者)为5440例,最终对资料完整的2043例女性进行统计分析。根据2010~2011年度体检时率压乘积水平,按照四分位数将研究对象分为四组:第一分位组为率压乘积<7500.00,第二分位组为7500.00≤率压乘积<8624.00,第三分位组为8624.00≤率压乘积<10121.00,第四分位组为率压乘积≥10121.00。多因素Logistic回归分析不同静息心率与收缩压乘积水平对Ba PWV的影响。结果 (1)2043例女性,平均年龄52.2±9.7岁,平均Ba PWV为1488.9 cm/s;(2)四组平均Ba PWV分别为1266.0、1377.3、1525.6和1786.7 cm/s;Ba PWV≥1400 cm/s的检出率分别为19.5%、37.2%、59.6%、84.3%;(3)影响Ba PWV的多因素Logistic回归分析显示:校正了年龄等因素后,相比于第一分位组静息心率与收缩压乘积,第二、第三和第四分位组Ba PWV≥1400 cm/s风险均增加,其OR值分别为1.66(95%CI 1.16~2.37)、3.18(95%CI 2.19~4.62)和7.30(95%CI 4.67~11.42)。结论静息心率与收缩压乘积是中老年女性Ba PWV增加的独立危险因素。Aim To investigate the relationship between rate-pressure product and brachial-ankle pulse wave velocity( Ba PWV) in middle-aged and older women. Methods A total of 5852 participants was selected with stratified random sampling from the 101510 workers of Tangshan Kailuan company who received the health examination. Among them 2043 women with integral data were recruited into the survey. The observation population was divided into four groups according to the rate-pressure product collected during 2010 ~ 2011 health examinations: quartile 1( the rate-pressure product < 7500. 00),quartile 2( 7500. 00 ≤the rate-pressure product < 8624. 00),quartile 3( 8624. 00 ≤the ratepressure product < 10121. 00),quartile 4( the rate-pressure product≥10121. 00). Multivariate logistic regression analysis was used to analyze the the influence of the rate-pressure product on Ba PWV. Results( 1) There were 2043 women with an average age of 52. 2 ± 9. 7 years old. Their mean of Ba PWV was 1488. 9 cm / s.( 2) The mean of Ba PWV for the above quartiles of the rate-pressure product were 1266. 0,1377. 3,1525. 6,1786. 7 cm / s,respectively; and the detection rates of Ba PWV≥1400 cm / s were 19. 5%,37. 2%,59. 6%,84. 3%,respectively.( 3) Multiple logistic regression analysis showed that after adjusting for age and other risk factors,compared with quartile 1,the risk of increasing Ba PWV was increased in quartile 2,3 and 4 of the rate-pressure product and the OR values were 1. 66( 95% CI 1. 16 ~2. 37),3. 18( 95%CI 2. 19 ~ 4. 62) and 7. 30( 95%CI 4. 67 ~ 11. 42),respectively. Conclusion The rate-pressure product is positively associated with increasing Ba PWV in middle-aged and older women.
