乡村医生签约式服务模式推行的现状分析:基于大丰市试点调查

摘　　要：目的:以江苏省大丰市为例,分析乡村医生签约式服务开展的现状、效果、及存在的问题进行分析,为促进乡村医生签约式服务的推行提供政策建议。方法:收集该市实施乡村医生签约的政策文件及2013—2014年相关数据,开展相关人员访谈了解签约开展现状。结果:至2014年3月大丰市共有69个村卫生室开展签约式服务,户签约率达到12.37%,虽然签约居民满意度高,但签约居民对签约服务的了解不够,不能有效参与签约服务的监督;开展签约式服务的村卫生室的参合农民人均就诊次数增长幅度比未开展的村卫生室高出15.85%,签约服务一定程度上引导农村居民在村卫生室接受治疗。结论:签约式服务有利于提高医疗服务的合理利用,高血压患者血压得到了控制,但需加大宣传、强化考核、加大培训、试点自由签约以促进签约服务的有效运行。Objective:To analyze the current situation,effects and existing problem of implementing rural doctor contract servicein Dafeng of Jiangsu,provide policy suggestions for the promotion of rural doctor contract service.Methods:Related policy docu-ments of implementing rural doctor contract service and data from 2013 to 2014 were collected and interviews were implemented forstudying the contract situation.Results:Until March 2014,there were 69 village clinics in Dafeng carried out the contract service,the signing rate reached to 12.37% by household, resident satisfaction was high,but the understanding of the contracted residents forthe contract service was insufficient, could not effectively attend the supervision for contract service;the growth rate of health clinicvisits per capita in the action-clinics was 15.85% higher than the no-action. To a certain extent, the contract service guided the ruralresidents to get treatment at the village clinic.Conclusion:The contract service was helpful for improving the rational use of medicalservices and the control of the blood pressure for patients with hypertension. However,it still needed to broaden the propaganda,en-hance the assessment,promoting the training and free contract in pilots so as to promote the effective operation of contract services.
