超声监视肠镜下阑尾腔内冲洗术治疗急性阑尾炎的临床疗效

摘　　要：目的探讨体外B型超声监视肠镜下阑尾腔内冲洗术治疗急性阑尾炎的临床效果。方法回顾性分析2013年6月至2014年12月经解放军白求恩国际和平医院消化内科诊断为急性阑尾炎的18例患者临床资料,在体外B型超声监视下行肠镜阑尾腔内冲洗术,总结分析临床资料、肠镜下的操作情况以及术后随访情况。结果 18例患者均成功接受了超声监视下肠镜阑尾腔内冲洗术,其中3例患者合并阑尾周围脓肿导致阑尾腔插管困难,经阑尾脓肿开窗术后插管成功。18例患者均见粪石排出,2例患者见铸型粪石排出,13例可见白色脓液或絮状物排出。操作过程均无并发症发生。术毕患者腹痛评分较术前均明显下降,术后3 d及1个月复查腹部超声较术前阑尾直径及壁厚均缩小。随访3~18个月均无复发。结论体外B型超声监视肠镜下阑尾腔内冲洗术治疗急性阑尾炎是安全有效的。Objective To investigate the curative effect of endoscopic intracavity douching in treatment of patients with acute appendicitis monitored by ultrasound. Methods Clinical data of 18 patients with acute appendicitis treated with endoscopic intracavity douching in treatment under the surveillance of external ultrasound from 2013 June to 2014 December were retrospectively analyzed. The colonoscopy and clinical manifestations and follow-up situations in patient were observed. Results All the 18 patients were successful operated,among whom 3 patients with preappendicular abscess treated by fenestration. Fecalith were discharged from eighteen patients,and proplasm fecalith were discharged from two patients,and pus and floccule were discharged from thirteen patients. No complication was observed in the whole operation procedure. After operation, the abdominal pain scores significantly decreased,and the postoperative ultrasonography showed that diameter and wall thickness of appendixes were shorter and thinner than that before operation. There was no recurrence during a follow-up 3-18 months. Conclusion Endoscopic intracavity douching in treatment of acute appendicitis under the surveillance of ultrasound is effective and safe.
