气压泵联合置管溶栓术治疗亚急性下肢深静脉血栓的疗效分析

摘　　要：目的探讨气压泵(IPC)联合置管溶栓术(CDT)治疗亚急性下肢深静脉血栓形成(DVT)的临床疗效。方法 2012年4月至2015年4月共84例亚急性下肢DVT患者随机分为对照组(单纯CDT组)和实验组[CDT加用间隙性气压泵组,CDT/IPC组)],各42例,两组均先给予下腔静脉滤器保护下CDT。对比两组患者溶栓时长、术后症状改善率及术后随访结果。结果 CDT/IPC组与单纯CDT组相比,溶栓时长明显降低[(62.14±13.97)小时vs(73.85±13.89)小时,P<0.05],术后症状改善率提高[95.24%vs 78.57%,P<0.05]。CDT/IPC组与单纯CDT组的血栓溶解率分别为52.38%和45.23%,术后6个月血栓形成后综合征(PTS)发生率分别为16.67%和21.43%,二组比较差异无统计学意义(P>0.05)。结论 IPC联合CDT治疗亚急性下肢DVT疗效显著,可有效减少溶栓时长,并改善症状。Objective To analyze the clinical effects of intermittent pneumatic compression(IPC)devices and catheter-directed thrombolysis(CDT) in the treatment of patients with subacute deep vein thrombosis(DVT) of lower limbs.Methods From April 2012 to April 2015,84 cases with subacute DVT of lower limbs were randomly divided into 2 groups(CDT group,CDT/IPC Group).All of the patients accepted CDT under the protection of inferior vena cava filter(IVCF).The duration of thrombolysis,improvement rate of postoperative symptoms and follow up outcomes were reviewed between two groups.Results Compared with CDT group,the duration of thrombolysis reduced [(62.14±13.97)h vs.(73.85±13.89)h,P<0.05] and improvement rate of symptoms increased [95.24% vs.78.57%,P<0.05] in CDT/IPC group.The rates of thrombolysis in CDT/IPC group and CDT group were 57.14% and 45.23%,respectively.The rates of post-thrombotic syndrome 6-month after procedure in CDT/IPC group and CDT group were 21.95% and 26.82%,respectively(P>0.05).Conclusion The IPC device combined with CDT in treatment of patients with subacute DVT of lower limbs is effective,with short duration of thrombolysis and great improvement of symptoms.
