自体骨髓干细胞移植治疗严重下肢缺血性疾病疗效的影响因素分析

摘　　要：目的探讨自体骨髓干细胞移植术治疗下肢严重缺血性疾病疗效的影响因素。方法回顾性分析本院2006年1月至2015年5月期间36例严重下肢缺血患者行自体骨髓干细胞移植治疗的临床资料,按缺血原因、缺血肢体远端输出道情况、缺血速度和年龄4个可能影响疗效的因素分组。结果缺血原因分组,动脉硬化性闭塞症组有效率33.3%(3/9),血栓闭塞性脉管炎组77.8%(14/18),糖尿病足组66.7%(6/9);远端输出道分组,输出道达小腿中段组有效率27.2%(3/11),踝关节76.9%(10/13),踝关节以下83.3%(10/12);缺血速度分组,急性缺血组有效率为0(0/4),慢性缺血组71.8%(23/32);年龄分组,≤60岁78.2%(18/23),>60岁38.4%(5/13),差异均有统计学意义(P<0.05)。结论自体骨髓干细胞移植术治疗下肢重度缺血性疾病的疗效与缺血原因、下肢的输出道、缺血的急慢性和年龄等因素有一定关系。Objective To investigate the influence factors of autologous bone marrow mononuclear cells transplantation(ABMMCT) in treatment of critical limb ischemia(CLI).Methods The data of 36 patients with CLI who were treated by ABMMCT from January 2006 to May 2015 were analyzed retrospectively.These patients were divided into four groups according to ischemic cause,outflow tract of lower limb,ischemic speed and age.Results Based on ischemia cause,efficient rate of CLI was 33.3%(3/9) in patients with arteriosclerosis obliterans(ASO),77.8%(14/18) in thromboangiitis obliterans(TAO)and 66.7%(6/9) in diabetic foot,respectively(P<0.05).The efficient rate was 27.2%(3/11),76.9%(10/13)and 83.3%(10/12)in the patients with outflow tract reaching the middle of the calf,ankle and below the ankle,respectively(P<0.05).The efficient rate was 0(0/4)and 71.8%(23/32) in patients with acute ischemia and chronic ischemia,respectively(P<0.05).The efficient rate was 78.2%(18/23) and 38.4%(5/13) in patients with ≤60 and >60 years old,respcetively(P<0.05).Conclusion The outcomes of ABMMCT in treatment of CLI is associated with ischemic cause,distal outflow tract,ischemic speed,age and so on.
